Visa Matter Request at IR,
REMARKS: This service is available only for foreign residents and fellows under Postgraduate Education Division, Faculty of Medicine Siriraj Hospital.
For Master Degree and Ph.D. Students, please contact Ms. Lalita Adunkittiphaisan, Student Affairs Section, Faculty of Graduate School, Mahidol University Salaya Campus call 0-2441-4125 Ext. 601.
WHAT CAN WE DO?
Dear All,
We just launch the new One-Stop Service Form for Visa Matter Requests. (i.g. visa extension, change of visa).
Step 1. Please complete One Stop Service Request Form (Form B).
Step 2. Please ask your supervisor or Head of Dept to sign at
Authorized Person in Section III.
Step 3. Please enclose a copy of passport on following pages;
(1) Front Page
(2) Visa Page
(3) TM.6 – departure card
Step 4. Submit your request document to IR Office.
Step 5. Collect visa document at IR Office in the next 2 working days.
***It will take 1-2 days to get a letter signed from the dean office***
