On September 30th, 2020, Prof. Dr.Prasit Watanapa, Dean of Faculty of Medicine Siriraj Hospital, Mahidol University, along with Prof. Dr.Prasert Auewarakul , Deputy Dean for Research and Siriraj Center of Research Excellence Management (SiCORE-M) led by Assoc.Prof.Sith Sathornsumetee, Manager visited the Thailand Science Research and Innovation (TSRI). A very warm welcome was given by Assoc. Prof. Dr.Patamawadee Pochanukul Acting President of TSRI ,Prof. Suthiporn Jitmitraparb, Former President, TSRI’s Administrative Members and staffs.

In the meeting, we had an honor to meet Dr. Nopporn Cheanklin (Executive Director, Health Systems Research Institute: HSRI) and Prof.Dr. Sansanee Chaiyarot (Health and Medical Subcommittee, Program Management Unit – Competitiveness: PMUC), two representatives of Program Management Unit (PMU). This meeting brought us an opportunity to provide brief introduction of our SiCORE-M unit and discussion on the possibilities of any potential future cooperation with authorities under Science Research and Innovation Systems.

On this occasion, Prof. Dr.Prasit Watanapa revealed that Faculty of Medicine Siriraj Hospital have been working on the research transformation process. SiCORE-M Unit was founded in November 2017 as a management initiative to (1) generate a cooperative ecosystem and appropriate research management, (2) increase tangible research outputs, (3) building research network and strengthening research capacity and (4) evolving the Faculty’s research administration and management. Not only responding to University’s or Faculty’s research policies but we also hope that this working approach would allow us to be associated with the new Science Research and Innovation Systems to effectively respond to Social and National policies.

During the meeting, Assoc. Prof. Dr.Patamawadee described about the restructuration of the National Science Research and Innovation Systems. As a result, the Thailand Research Fund (TRF) that had been originally designated to support the research fund for each project, they have become TSRI instead. TSRI are in charge of strategic planning, allocation of research fund and other responsibilities that are work-related or being assigned. One of the significant objectives is to raise Thailand investment in research fund to 2% of GDP within 2027. Recently, our knowledge management team (KMT Think tank) has been established to perform evidence based research on the guideline to TSRI for research funding allocation as well as the prioritization based on research outcomes and impact at national level, more importantly the solution for the problem of our country need to be addressed. In the medical dimension, the KMT Think tank team had created a project study called ” the guidelines for research product prioritization of the advanced therapy medicinal product” in order to set the priority of research theme.

Therefore, the potential for medical development should be aligned with the National Strategy and the “BCG Model” policy, which includes economic development in terms of Bioeconomy, Circular Economy and Green Economy simultaneously. In addition, research budget allocation (in percentage/proportion) should be considered and used for operation plan setting which is for research medical technology development and public health. The purpose is to attain outcomes and impact in accordance with the TSRI’s scheme and Assoc. Prof. Dr.Pongsakorn Tantilipikorn is designated to be one of the CMT Medical committee.

On the final note, SiCORE – M are enthusiastic to cooperate with the units under Science Research and Innovation Systems through the two mechanisms for funding allocation as following;

1.) Fundamental Fund, which will be directly allocated to the university

2.) Strategic Fund that will be distributed to other PMU. The supporting PMU for Medical consist of Health Systems Research Institute (HSRI), Program Management Unit – Competitiveness (PMUC) and Program Management Unit – Brain Power (PMUB).

The original research paper from Siriraj Center of Research Excellence for Cancer Immunotherapy (SiCORE-CIT) entitled “Anti-Tumour Effect of the Fourth-Generation Chimeric Antigen Receptor T Cells Targeting CD133 Against Cholangiocarcinoma Cells” by Ms. Thanich Sangsuwannukul (First Author) and Prof. Pa-thai Yenchitsomanus (Corresponding Author) was accepted to be published in ELSEVIER‘s International Immunopharmacology!

The “International Immunopharmacology” is the primary publication of original research papers pertinent to the overlapping areas of immunology, pharmacology, cytokine biology, immunotherapy, immunopathology, and immunotoxicology.

Fluid Therapy was a standard treatment for patients with septic shock. But, recent retrospective evidence suggests the efficacy of early norepinephrine administration during resuscitation. However, there was limited information from prospective randomization trials to support this postulation.

The research was set up to conduct a Randomized Control Trial to evaluate the hypothesis that early low-dose norepinephrine in adults with sepsis with hypotension increases shock control compared with standard care.

The results of this clinical trial demonstrated a significant association between early norepinephrine and increased shock control. Further studies are needed to confirm these findings before this approach can be introduced in clinical resuscitation practice. Future studies should investigate the effect of early norepinephrine on organ dysfunction and mortality.

 

Cholangiocarcinoma (CCA) remains a serious public health concern in Thailand due to the highest incidence and mortality rate in the world, as high as 6,000 – 10,000 fatalities/year! The patients usually die in distress within 6 months after detection due to a lack of effective pharmacological treatment. Currently, the only effective treatment is surgical treatment but can perform in a small number of patients. So, the research of effective medications for the patient is extremely important and should be prioritized first.

There are many Cholangiocarcinoma cases in Thailand, around 8 out of 10 Cholangiocarcinoma cases in the World!. There’s a lot of effort to prevent and to surveillance, the number of patients for the past decade but the number of cases is unlikely to decline. So, researching for an effective treatment is one of the key elements to the fight against this disease.

This is the collaboration between the Faculty of Medicine Siriraj Hospital and Faculty of Medicine, Khon Kaen University. This recent discovery in CCA research has shown magnificent results and give some hope for Cholangiocarcinoma patients. The “CDK4-Cyclin D1 inhibitors” such as Palbociclib, Ribociclib, and Abemaciclib are effective and already been used in the clinic for several types of cancer i.e. breast cancer, with lower toxic compare to other chemotherapeutic agents. The Palbociclib, Ribociclib, and Abemaciclib are usually conveniently taken orally once a day, with relatively safer profiles compared to the chemotherapeutic agents. This is a giant leap in the Cholangiocarcinoma treatment.

Educational assessment and evaluation is a key component in improving the quality of education development by obtaining the necessary data to evaluate the clinical student that they have achieved the learning objectives and standards or not? This is directly beneficial to the learners and also reflects the teaching efficiency of the teachers including the quality of educational management in the institutions. The educational assessment and evaluation of learners in the “Doctor of Medicine” program is different from other programs, especially at the clinical level, with the specifically and variously assessment and evaluation formats such as Multiple-Choice Question, Modified Essay Question, Oral Examination, Clinical Practice Examination, Objective Structured Clinical Examination, Assessment of Personality through Behavioral Observations in Work Simulations, Group Work Assessment, and Portfolio Assessment. The person who plays an important role in the clinical students’ assessment and evaluation is a clinical teacher. But, there’re still problems that some clinical teachers in the medical schools, university-affiliated hospitals, and clinical medical education centers still lack knowledge and understanding in the principles of clinical-grade medical students’ assessment and evaluation, including the standard for assessment and evaluation of each clinical teacher’s isn’t the same standard. The objectives are to learn the meaning, character, condition, and experience of a doctor’s spirit. To understand the process of “How to create a doctor with a doctor’s spirit. The activities consisted of a 2-hour lecture by Ms. Phachadapan Odompet, Ph.D., an educator from Division of Medical Education, Faculty of Medicine Siriraj Hospital, Mahidol University (simultaneously learning both in the classroom and live broadcast via social media and Mahidol University’s IPTV channel). The lecturers are from Siriraj and other institutes such as medical schools, university-affiliated hospitals, clinical medical education centers, and health science institutes. The outcome can be seen as the Knowledge Transfer to the public.
